Before filling out a registration form, check you are in our catchment area using the map and postcode checker opposite.
If you move outside this area permanently you will need to register with another doctor.
If you move and remain inside the Practice area, please immediately inform us of your new address and telephone number.
Registering with the surgery
To register with the surgery:
- fill out an online registration form AND a health questionnaire
- visit reception and collect a paper registration form
You will be advised which of our doctors is currenlty taking on new patients. The reception staff are there to help you if you have any queries.
You may see any available doctor. For continuity of care, it may be helpful to see the same doctor every time, especially for on-going problems. In this way you can get to know your doctor, and the doctor will become familiar with your particular illness and circumstances.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Access
The building is suitable for disabled access, including wheelchairs. We have spacious toilet facilities for both sexes off the waiting room and adjacent to the Nurses' treatment rooms.

NEW PATIENT REGISTRATION FORM
Register as New Patient
Please be aware that we are currently experiencing a high volume of registrations and this may take some time to complete. We would therefore recommend that you order your medication from your existing GP practice in order to make sure you do not go without your medication.
Fields marked "REQUIRED" are compulsory. You should only send this form if you are sure that you are eligible to join this practice. Sending this form will NOT automatically register you with the surgery. Your details will be held at the surgery for a limited period of time. You are required to present in person to sign your registration form and provide proof of your address. Sending this form does NOT guarantee or even imply that you will be accepted onto the practice register.

New Patient Registration
Northlands surgery is accepting new patients. You can register with the practice by scanning the QR code below or click on the link to the right
Temporary Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Accountable GP
All patients have a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients may see any GP or nurse in the practice.
Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access:
- collect a proxy access form from reception
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
